ABSTRACT

Objetivo: Com o presente estudo, buscou-se verificar a quantidade de exames de colpocitologia oncótica coletados em Unidade Básica de Saúde (UBS) escolhida, determinar quantas e quais são as alterações encontradas nesses exames e quais foram as condutas e acompanhamentos realizados em pacientes com exames alterados. Métodos: Foram analisados 446 exames de colpocitologia oncótica realizados em uma UBS do centro-oeste do Paraná. As pacientes com exame alterado foram selecionadas e foi feita uma análise desses prontuários a fim de verificar a conduta e o acompanhamento dessas pacientes. Resultados: Foram encontrados 32 exames com alterações, sendo três de uma mesma paciente. Dessas 30 pacientes, a grande maioria correspondia a células escamosas atípicas de significado indeterminado (ASC-US), possivelmente não neoplásicas, e 28 pacientes retornaram para receber uma conduta inicial, sendo principalmente o encaminhamento para a coleta de novo preventivo em seis meses ao serviço de oncologia clínica. Conclusão: O presente estudo concluiu que a hipótese principal foi verdadeira, a qual foi descrita como prevalência significativa de lesões intraepiteliais de alto grau, embora não seja a mais frequente, e descontinuidade por parte das pacientes que obtiveram esse resultado. Além disso, notou-se grande encaminhamento à atenção terciária, o que não é indicado pelo Ministério da Saúde.(AU)


Objective: With the present study, we sought to verify the amount of cytopathological tests collected in the chosen basic health unit, to determine how many and what are the alterations found in these tests and what were the conducts and follow-ups performed in patients with altered exams. Methods: We analyzed 446 preventive tests performed in a basic health unit in the Midwest of Paraná, and patients with altered examination were selected and an analysis of these medical records was made in order to verify the conduct and follow-up of the patients. Results: Thirty-two tests with alterations were performed, 3 of which were the same patient. Of these 30 patients, the vast majority corresponded to atypical cells of squamous meaning ­ possibly non-neoplastic (ASC-US), and 28 patients returned to receive an initial approach, mainly being referred to the collection of preventive new in 6 months and to the clinical oncology service. Conclusion: The present study concluded that the main hypothesis was true, which was described as a significant prevalence of high-grade intraepithelial lesions (HSIL), although not the most frequent, and discontinuity on the part of the patients who obtained this result. In addition, there was a large referral to tertiary care, which is not indicated by the Ministry of Health.(AU)

ABSTRACT

La falta de cultura preventiva y un diagnóstico precoz oportuno representan dos de los factores más importantes a considerar en la prevención y tratamiento de las enfermedades crónicas no transmisibles a nivel mundial, por lo que la tecnología móvil constituye una alternativa que bien puede ser utilizada en la medicina preventiva. En este sentido, el presente artículo tuvo como objetivo validar una aplicación tecnológica que permita realizar el diagnóstico de enfermedades crónicas no transmisibles, utilizando el dispositivo móvil. Se realizó un estudio descriptivo, que utilizó la encuesta como técnica y el cuestionario como instrumento para la recolección de la información en una muestra de 60 personas mayores de 18 años, todos usuarios de la red social Facebook, quienes seguían la página https://es-la.facebook.com/DrLuisPacoraCamargo/. Los resultados obtenidos permitieron demostrar que el 57,1 por ciento de los encuestados presentaba edades comprendidas entre los 21 y 40 años de edad, de los cuales el 88,3 por ciento reportó que utilizaba internet diariamente; el 53,3 por ciento respondió que utilizaba sus dispositivos frecuentemente para comunicarse a través de las redes sociales; el 53,4 por ciento de los encuestados había descargado un aplicativo relacionado con la salud y, finalmente, el 82,8 por ciento de las personas respondió tener conocimiento del término de medicina preventiva y, que de tener información en sus dispositivos móviles, la utilizaría con frecuencia. En conclusión, el diseño e implementación de un servicio de salud a través de los dispositivos móviles, como el CHECKEATE, es viable en un mercado joven, potencialmente interesado en temas de salud preventiva(AU)


Lack of a preventive culture and of timely early diagnosis are two of the most important factors to be considered in the prevention and treatment of chronic non-communicable diseases worldwide. Mobile technology is therefore an alternative which could very well be used in preventive medicine. The purpose of the study was to validate a technological application for the diagnosis of chronic non-communicable diseases using a mobile device. A descriptive study was conducted based on the survey as technique and the questionnaire as data collection tool. The sample was 60 people aged over 18 years, all of them users of the social network Facebook, who follow the webpage https://es-la.facebook.com/DrLuisPacoraCamargo/. Results show that 57.1 percent of the respondents were aged 21-40 years; 88.3 percent reported using the Internet daily; 53.3 percent that they used their devices frequently for communication through social networks; 53.4 percent had downloaded a health-related application, and 82.8 percent were familiar with the term "preventive medicine" and would used it more frequently if they had more information on their mobile devices. In conclusion, design and implementation of a health service through mobile devices, as is the case with CHECKEATE, would be viable in a young market potentially interested in preventive health topics(AU)

ABSTRACT Objective: To apprehend the social representations elaborated by older people about HIV/AIDS and to understand how they relate to the prevention of HIV infection. Method: Descriptive and qualitative research based on the Theory of Social Representations with 42 older people assisted at primary care. Data were produced through in-depth interviews with a semi-structured instrument, processed in the IRaMuTeQ software, and analyzed by means of the descending hierarchical classification. Results: Five classes emerged: "HIV/AIDS: a problem of young people"; "Quality of life improvement for people living with HIV/AIDS"; "Vulnerability to HIV/AIDS among heterosexual women in a stable union"; "HIV/AIDS Information Network: process of creation and transformation of social representations" and "Prevention versus stigma". Final considerations: The social representations that older people have about HIV/AIDS influence the adoption of preventive measures negatively because stigma is present and HIV/AIDS is attributed to young men, and to men who have sex with other men.


RESUMEN Objetivo: Comprender las Representaciones Sociales elaboradas por ancianos sobre el VIH/SIDA y entender cómo se relacionan con la infección por VIH. Método: Investigación descriptiva, cualitativa, fundamentada en la Teoría de las Representaciones Sociales, con 42 ancianos atendidos en atención primaria. Datos recolectados mediante entrevistas en profundidad aplicando instrumento semiestructurado, procesados en software IRaMuTeQ y analizados por Clasificación Jerárquica Descendente. Resultados: Surgieron cinco clases: "VIH/SIDA: un problema de jóvenes"; "Mejora de calidad de vida de personas que viven con VIH/SIDA"; "Vulnerabilidad al VIH/SIDA de mujeres heterosexuales en pareja estable"; "Red de informaciones sobre VIH/SIDA: Proceso de creación y transformación de las Representaciones Sociales"; y "Prevención versus Estigma". Consideraciones finales: Las representaciones sociales que los ancianos tienen respecto del VIH/SIDA influyeron negativamente a la adopción de medidas preventivas, pues el estigma está presente y el VIH/SIDA es atribuido a jóvenes y hombres que tienen sexo con otros hombres.


RESUMO Objetivo: Apreender as Representações Sociais elaboradas por idosos sobre o HIV/Aids e compreender como elas se relacionam com a prevenção da infecção pelo HIV. Método: Pesquisa descritiva e qualitativa fundamentada na Teoria das Representações Sociais, com 42 idosos atendidos na atenção primária. Os dados foram produzidos por meio de entrevistas, em profundidade, com um instrumento semiestruturado, processados no software IRaMuTeQ e analisados pela Classificação Hierárquica Descendente. Resultados: Emergiram cinco classes: "HIV/Aids: um problema de jovens"; "Melhoria da qualidade de vida de pessoas vivendo com HIV/Aids"; "Vulnerabilidade ao HIV/Aids de mulheres heterossexuais em união estável"; "Rede de informações sobre HIV/Aids: Processo de criação e transformação das Representações Sociais"; e "Prevenção versus Estigma". Considerações Finais: As representações sociais que os idosos têm sobre o HIV/Aids influenciam de forma negativa na adoção de medidas preventivas, pois o estigma está presente e o HIV/Aids é atribuído a jovens e homens que fazem sexo com homens.

ABSTRACT Objective: To identify the knowledge and self-care actions taken by nursing undergraduate students of a Federal University of the South of Brazil, against Sexually Transmitted Infections. Method: Exploratory qualitative study, conducted 40 interviews with undergraduate students at the beginning and end of the course. The analysis was thematic, resulting in three categories. Results: Knowledge about the subject is a decisive factor for self-care, and the more knowledge, the greater the prevention. The dissemination of knowledge of students at the end of the course not only influences self-care but also health promotion in the social sphere. Final considerations: Knowledge is important in self-care and caring for others. The dissemination of knowledge becomes evident according to the complexity of the course. Stable relationships may interfere with the use or disuse of condoms in sexual relationships, a misnomer present in today's society.


RESUMEN Objetivo: Identificar el conocimiento y las acciones de autocuidado tomadas por estudiantes de graduación en Enfermería de una Universidad Federal del Sur de Brasil frente a las Infecciones de Transmisión Sexual. Método: Estudio cualitativo exploratorio. Fueron realizadas 40 entrevistas con estudiantes de graduación del inicio y del final del curso. El análisis fue temático y resultó en tres categorías. Resultados: El conocimiento acerca de la temática es un factor decisivo para el autocuidado, considerando que cuanto más conocimiento, mejor es la prevención. La diseminación de conocimientos de los estudiantes de final del curso no sólo influye en el autocuidado, sino también en la promoción de la salud en el ámbito social. Consideraciones finales: El conocimiento es importante en el autocuidado y en el cuidado al prójimo. La diseminación de conocimientos se hace evidente según la complejidad del curso. Las relaciones estables pueden interferir en el uso o desuso de los preservativos en las relaciones sexuales, equivocación presente en la sociedad actual.


RESUMO Objetivo: Identificar o conhecimento e ações de autocuidado tomadas por estudantes de graduação em Enfermagem de uma Universidade Federal do Sul do Brasil, frente às Infecções Sexualmente Transmissíveis. Método: Estudo qualitativo exploratório, realizado 40 entrevistas com estudantes de graduação do início e do final do curso. A análise foi temática, resultando em três categorias. Resultados: O conhecimento acerca da temática é um fator decisivo para o autocuidado, sendo que quanto mais conhecimento, maior a prevenção. A disseminação de conhecimentos dos estudantes de final do curso não só influenciam no autocuidado como também na promoção de saúde no âmbito social. Considerações finais: O conhecimento é importante no autocuidado e no cuidado ao próximo. A disseminação de conhecimento se torna evidente conforme a complexidade do curso. Os relacionamentos estáveis podem interferir no uso ou desuso dos preservativos nas relações sexuais, equivoco presente na sociedade atual.

ABSTRACT

Resumen: La teoría de la medicina supresora (MS) y la medicina estimulante (ME) destaca dos formas radicalmente distintas de entender la enfermedad y de enfrentarla. Para la MS es un objeto extraño o ajeno al organismo que hay que disminuir o destruir (suprimir); para la ME es un trastorno de la armonía interna que requiere fortalecer o avivar el organismo para restablecerla (estimular). En la medicina moderna, el poder de la industria de la salud favorece el predominio de la MS, por su alta rentabilidad, y la marginación de las medicinas alternativas (MA), poco rentables que, al igual que la vacunación o las terapias sustitutivas y regenerativas, son formas de ME. El efecto placebo (EP) inherente a la práctica médica, revelador de fuerzas curativas endógenas susceptibles de estimulación, da sentido a la ME y credibilidad a las MA. La dirección del EP de lo macro (psicosocial) a lo micro (físico-químico) explica su alta especificidad y ausencia de efectos secundarios. El efecto farmacológico de lo micro a lo macro, opuesto a las fuerzas endógenas, conlleva indefectiblemente efectos secundarios que requieren ulteriores supresiones y dosis repetidas indefinidamente. Se analizan equívocos de la ciencia con respecto al EP y al imponer a las MA criterios metódicos propios de la MS, que las desvirtúa, descalifica y excluye como objetos de conocimiento. Se insiste en la necesidad de reconocer la ME y rescatar las MA para la indagación, a fin de explorar sinergias, complementos o reemplazos con relación a la MS en la búsqueda del bien vivir.


Abstract: The theory of suppressive (SuM) and stimulant (StM) medicine highlights two radically different ways of understanding and coping with diseases. For SuM it is a strange or foreign object to the organism that must be diminished or destroyed (suppressed); for StM it is a disorder of internal harmony that requires strengthening or enlivening the body to re-establish it (stimulate). In modern medicine, the power in the health industry favors the predominance of SuM, because of its high profitability, and the marginalization of low-cost alternative medicines (AM) that, like vaccination or substitutive and regenerative therapies, are forms of StM. The placebo effect (PE) inherent to medical practice, revealing of endogenous curative forces susceptible to stimulation, gives meaning to StM and credibility to AM. The direction of the PE from the macro (psychosocial) to the micro (physical-chemical) explains its high specificity and absence of side effects. The pharmacological effect of the micro to the macro, opposed to the endogenous forces, inevitably entails side effects that require further suppression and indefinite repetition of doses. Scientific assertions that misunderstand PE, and impose on the AM methodical criteria of the SuM that detract, disqualify and exclude them as objects of knowledge, are analyzed. The emphasis is on the need to recognize the StM and rescue the AM for inquiry in order to explore synergies, complements or replacements in relation to the SuM, in the quest for to live well.

ABSTRACT

El presente artículo pretende inducir a una discusión crítica acerca de las nociones conceptuales de la prevención aplicados al fenómeno del consumo de sustancias psicoactivas. Se realizó un recorrido teórico sobre las nociones acerca de la enfermedad y como estas preceden el concepto de prevención. De la misma se abordan aproximaciones al término de prevención y algunos de los modelos existentes, los cuales han sido bandera de las ciencias de la salud y finalmente estableciendo puntos de encuentro y desencuentro, lo cual abre un espacio necesario para que las ciencias sociales realicen sus portaciones. Finalmente, se discute sobre la imposibilidad de la prevención bajo el marco conceptual y metodológico existente.


This article aims to induce a critical discussion of the conceptual notions of prevention applied to the phenomenon of psychoactive substances. A theoretical course on notions about the disease was made and how are you precede the concept of prevention. Similarly the term prevention approaches are discussed and some of the existing models, which have been the projects of health sciences and finally establishing points of convergence and divergence, which opens up a space needed for the social sciences to make their contributions. Finally, there is the discussion about the impossibility of prevention under the existing conceptual and methodological framework.

Es indiscutible la relevancia de investigaciones realizadas en el ámbito de las enfermedades crónicas no trasmisibles y sus factores de riesgo como es el caso de la Diabetes Mellitus (DM) y la aterosclerosis. El trabajo que de forma tan acertada la Dra. Mayra Agramonte Martínez pone a nuestra consideración; Prevalencia de factores de riesgo de aterosclerosis en pacientes diabéticos tipo II, 1 es un ejemplo del afán del Estado Cubano y los profesionales de la salud de elevar la calidad de vida de la población. En el mundo actual, los hábitos alimentarios inadecuados y tóxicos, y los estilos de vida sedentarios condicionan positivamente la aparición y, cada vez, en edades más tempranas, de enfermedades crónicas no trasmisibles(AU)

ABSTRACT

OBJECTIVES: The aim of this study was to develop search filters able to retrieve the South Korean scientific output and relating the fields of public health, preventive medicine, and physical education, activity, fitness, exercise and sport in MEDLINE between 2000 and 2007. METHODS: The search filters were constructed and applied in MEDLINE accessed through PubMed according to the affiliation and subject. The language and place of publication were identified and the evaluation of the geographical filter performance was done. RESULTS: The search format was successfully elaborated and applied, and the articles originated, published in Korea and/or abroad focusing on the fields of public health, preventive medicine, physical education, activity, fitness, exercise and sport, added to MEDLINE between 2000 and 2007 were retrieved. Publications in six languages originated in South Korea were detected. CONCLUSIONS: A geographic search filter determined the South Korean research output, and combined with additional filters focused on specific areas. The dynamics of the scientific output followed an increased evolution in all categories. Articles were written mainly in English/Korean. Further research is recommended on developing search strategies in order to retrieve precise and reliable information.

ABSTRACT

Os atuais métodos de previsão de eventos cardiovasculares são muito imprecisos, somente explicando aproximadamente metade dos eventos. Isso é ainda mais acentuado no grupo de pacientes assintomáticos. A tomografia computadorizada por múltiplos detectores tem se mostrado um valioso aliado do cardiologista, fornecendo em um só exame, diversas informações sobre o sistema cardiovascular que, combinadas, têm o potencial de aumentar muito a acurácia na previsão de eventos cardiovasculares, diminuindo a morbimortalidade. No presente artigo será examinado o presente e potencial futuro desse novo método na avaliação de pacientes assintomáticos do ponto de vista cardiovascular.

ABSTRACT

OBJECTIVE: To assess school teachers' level of knowledge on prevention of viral hepatitis (VH). MATERIAL AND METHODS: A cross-sectional study was conducted in three cities of Brazil, from August to November of 1999. The sample was composed of 360 subjects: 334 women and 26 men, 81 (22.5 percent) from Belém, 123 (34.2 percent) from Natal and 156 (43.3 percent) from Rio de Janeiro. Cultural differences in knowledge were identified using a questionnaire to classify, according to semantic content, categories of transmission and preventive practices. Responses were scored as right or wrong. Data were tabulated and analyzed using EPIINFO 6.04 and open answers were classified according to semantic content. Comparison of the answer frequencies between cities was done through the chi-square test. RESULTS: Transmission category (TC) (n=837 answers) and prevention category (PC) (n=771 answers) "food-and waterborne" transmission items were the most frequently mentioned (40 percent). For TC, "food-and waterborne" answers were followed by "bloodborne" (16 percent), "inadequate knowledge" (9 percent), "possible causes of hepatic disease" (9 percent), and "sexual transmission" (7 percent) answers. For PC items, "food-and waterborne" answers were followed by "general aspects of prevention" (13 percent), "immunization" (9 percent), "quality of health services" (8 percent) and "sexual prevention" (5 percent) items. "Right" scores for transmission mechanisms and prevention practices varied from zero to 80 percent. CONCLUSIONS: Study findings suggest that investments should be made to disseminate appropriate knowledge on VH prevention, mainly addressing sexual transmission and intravenous drug use
